Personal Data Application Form

Under Law No. 6698 on the Protection of Personal Data (“KVKK”), data subjects (defined in the Law as “relevant persons” and hereinafter referred to as the “Applicant”) are granted the right to make certain requests concerning the processing of their personal data under Article 11 of the KVKK.
Pursuant to the first paragraph of Article 13 of the KVKK, applications to our Company as data controller concerning these rights must be submitted to us in writing or by the other methods determined by the Personal Data Protection Board (the “Board”).
How to Apply
In this context, applications made to our Company “in writing” may be submitted to us by printing out this form and delivering it:
- in person by the Applicant,
- through a notary public,
- by signing it with a “secure electronic signature” as defined in Electronic Signature Law No. 5070 and sending it to the Company’s registered electronic mail address.
Response Time
Pursuant to the second paragraph of Article 13 of the KVKK, your application will be answered within thirty (30) days from the date it reaches us, depending on the nature of the request. Our responses will be delivered to you in writing or electronically in accordance with Article 13 of the KVKK.

This application form has been drawn up so that your relationship with our Company can be established, any personal data processed by our Company can be identified in full, and your application can be answered accurately and within the statutory period. In order to eliminate legal risks that may arise from unlawful and unjust data sharing, and in particular to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of an identity card or driving licence) for the purpose of verifying identity and authority. Where the information relating to the requests you submit under this form is inaccurate or out of date, or where an unauthorised application is made, our Company accepts no liability arising from such incorrect information or unauthorised application.
